What is philtrum length?

Philtrum length is the vertical distance from the base of the nose (subnasale) to the top of the upper lip's vermilion border. Philtrum length naturally increases with age, contributing to the 'aging lip' appearance where the upper teeth become less visible. Lip lift surgery shortens the philtrum to restore youthful upper lip exposure.
How philtrum length is measured
Measured in millimeters from a front-facing or profile photograph. IPD calibration anchors the absolute measurement.
Why philtrum length matters
Philtrum length affects whether the upper teeth are visible at rest. Short philtrums show more upper teeth (often a youthful appearance); long philtrums conceal upper teeth (often an aging appearance). Lip lift surgery is the standard cosmetic procedure for philtrum shortening.
Normal range
Adult female: 11 to 15 mm. Adult male: 13 to 17 mm. The ratio increases by approximately 1 mm per decade after age 30.

Lip filler does not change philtrum length. Lip filler increases lip volume, which can visually de-emphasize a long philtrum but does not shorten it. The procedure that actually shortens the philtrum is lip lift surgery (also called bullhorn lift).
